COVID-19 VACCINATION NEW

We will be offering the government-paid Pfizer-BioNTech Covid-19 vaccine likely by mid-December. More details will be available here.

This will be offered on appointment basis via our National Appointment System (NAS) for Covid-19 vaccination once available.


COVID -19 PANDEMIC

All patients and visitors to our clinic are required to comply with the following entry requirements:

STEP 1. Scan and Declare (outside front-door)

  • Scan the QR Code outside the front-door using your phone's QR scanner (do not use TraceTogether or SingPass to scan - they are incompatible)
  • Patients are to select "See Doctor" and make the necessary Health Declaration
  • All others should select "Accompany Patient" or "Others" to check-in (SafeEntry)
  • Please wait outside the front-door till called in.

STEP 2. Tap your TraceTogether App or Device at our Front Counter (on entry) - 7 yrs old and above

  • TraceTogether App must be turned on. If you use a device instead, it must be kept by your side all the time. Our staff are required by MOH to check compliance. Kindly cooperate.

Please be assured that we will take all precautions and measures to ensure your safety during your visit to our clinic. We apologise for the inconvenience caused by all the above measures and trust that you will understand that they are for everybody's good.

Those without either TraceTogether App on their mobile phone or possess a TraceTogether Device will not be allowed into our premises wef 17 May 2021.

SWAB AND SEND HOME (SASH) FACILITIES

With effect from 1 July 2020, every person from the age of 13yrs old with symptoms suggestive of Acute Respiratory Infection (ARI) will be tested for Covid-19. This is only available Monday to Saturday, morning and some afternoon sessions. Children under the age of 13yrs old may also be tested if they fall into the "suspect case" category as defined by MOH.

With effect from 15 May 2021, Two tests will be conducted for each patient. An anterior nasal swab for ART and a nasopharyngeal swab for PCR. These tests are government funded for patients with ARI pr Fever with no known cause. Patients do not need to pay for them.

IF YOU ARE UNWELL WITH FEVER OR RESPIRATORY SYMPTOMS

  •     Please call us at Tel 64566582 to inform us of your condition, wear a mask and come preferably in the morning, from Monday to Saturday.
  •     If you have been given an MC for respiratory symptoms, you are required by law to stay at home for the duration of the MC or until you have been notified of a negative Covid-19 PCR test result.
  •     Do not doctor-hop. Always keep to the same clinic that you first sought treatment. If unavailable and in emergencies - please call 995 for an ambulance.

PRE-DEPARTURE COVID-19 TEST for TRAVELLERS /
PRE-EVENT COVID-19 RAPID ANTIGEN TESTS

Pre-departure and Pre-Event Covid-19 Swab test is now available at our clinic on appointment basis.

Before making an appointment with us, please check SafeTravel website and the embassy website of the country you wish to visit to obtain the latest information on requirements.

TRAVEL TO CHINA (NEW) - Travellers heading to China on or after 30 October 2021 must meet new requirements of doing 1 PCR Test 3 days before departure AND 1 PCR Test plus Serology at a different clinic Test 2 days before departure. Our swab and serology tests are done by Parkway Laboratories, one of the two designated test agencies.

To make an appointment with us for Pre-Departure tests, kindly email staff@familymedicare.com with the following information:

  • Traveller's name,
  • Traveller's nationality (citizenship)
  • NRIC or FIN number
  • Destination travelling to
  • Departure date and time
  • Mobile Phone Number
  • Current Address
  • Email Address (if different from the email address used to send particulars)
  • IMPORTANT: Attach scanned images of the following documents:
                      1) passport
                      2) air-ticket
                      3) NRIC/ FIN card (if available)
                      4) Covid-19 Vaccination Certificate (if available)

We will call/email you back by the next working day to confirm your appointment if all your documents are in order. For URGENT requests made less than 48hrs before flight, please call us at 64566582 during office hours or mark URGENT on your email if after hours. The following fees apply (depending on which test is required):

  • RT-PCR Swab Test - SGD $138.00 (GST included) (NEW) REDUCED PRICE
  • Serology (IgM) Blood Test - SGD $40.00 (GST included)
  • Serology (Anti-N Antibodies) - SGD $60.00 (GST included)
  • Rapid Antigen Test / Pre-Event Test (PET) - SGD $38.00 (GST included) - Results within half an hour of test  (NEW) REDUCED PRICE
  • Fitness to travel memo (no tests) - SGD $53.50 (GST included)
  • Test for Successful Overseas Immunisation (Anti-S Antibody) - SGD $60.00 (GST included). Please note that verification/updating into NIR incurs additional $18.00 (inclu GST).

These are not refundable and any retest is payable again in full. No consultation fees will be incurred unless further consultation is required (eg for other matters).

Swab results will be sent to you by email, usually by the next day, but this cannot be guaranteed. If you do not hear from us within 48hrs, please call us for assistance.

Walk-in for Pre-Event Covid tests are possible. Rapid Antigen Test results/certificates are available within half an hour of test and will be sent to your mobile phone.

Different countries have different timing requirements for tests to be performed. Changes can also be made without advanced notice. It is the responsibility of the traveller alone to ensure that these conditions are met.

Please understand that your Covid-19 test could be unsatisfactory and/or delayed for a variety of reasons. Any arrangements you make on the basis or assumption that your Covid-19 test will be satisfactory and/or not delayed will be entirely at your own risk. We shall not be responsible and/or liable in any circumstances whatsoever with respect to Covid-19 testing.

 

E-HealthCerts and Notarisation

Reports for RT-PCR (swab) and Serology (IgM) will be sent via email to you as locked .pdf and .json files. For security, these are locked. Use your NRIC or FIN number to unlock these files.

From 10 Mar 2021, HealthCerts have to be notarised before travel. This will be automatically done by our laboratory at no cost to you.
